Physical Information

All students have three requirements to be able to participate in athletics at Heritage Hills High School/Middle School for the 2023/2024 School Year:

1. Final Forms Log-In 

   1. Only needs to be done once per household

   2. How to Register

2. Final Forms Documents 

   1. First year form completion with yearly review/update as needed

   2. Final Forms Documents

   3. Print the "IHSAA PPE Phyiscal Form" to take with you to the provider

3. Pre-Participation Physical dated on or after April 1, 2023 

   1. New physical required for each school year

   2. Blank Physical Evaluation Form

   3. Can be turned in one of three ways:

      1. Uploaded as a PDF to Final Forms (preferred method)

      2. Dropped off to the Athletic Trainer in person or mailbox located outside the AT Room

      3. Given to coach or the Athletic Office

*Athletes will not be allowed to tryout if the both the physical and Final Forms Documents have not been completed. Once the physical and Final Forms Documents are completed, your child may try out for one or multiple sports teams at Heritage Hills High/Middle School.
